To ensure the rights and accuracy of shareholder information, we kindly request Shareholders to update your information (including Establishment License/Operating License/Business Registration Certificate/Securities Trading Code/Passport number, date/place of issue of the Establishment License/Operating License/Business Registration Certificate/Securities Trading Code/Passport, contact address, etc.) by following the procedures below:

Submission of Documents (In-person or via Express Mail)

Required Documents:

• Request for Change of Information (attached form): 02 originals.
  • In case Shareholders submit documents via express mail, the Request for Change of Information must have the shareholder's signature authenticated by a competent authority.
• Legal Documents:
  • For individuals:
    • Approval for securities trading code: 01 copy.
    • Passport: 01 copy.
    • In case of changing the passport number: Documents verifying previous information (e.g. old passport).
  • For organizations:
    • Approval for securities trading code: 01 copy.
    • Establishment License/Operating License/Business Registration Certificate: 01 copy.
    • Valid Power of Attorney (if any): 01 copy.
    • Passport of the legal representative and the authorized person: 01 copy.
    • Letter of Introduction from the organization for the staff, alongside the staff member's personal identification documents, when submitting the documents: 01 original.

Note:

• Copies must be certified as true and accurate against the original by a competent authority in Vietnam (or by a Vietnamese diplomatic representative office abroad).
• If documents are issued abroad, they must undergo consular legalization in accordance with the regulations of the Ministry of Foreign Affairs of Vietnam.
• Express mail packages must clearly state “PVComBank Shareholder Information Update” on the outside of the envelope.

Implementation Timeframe: from the date of receiving this notice until the end of December 25, 2025.
